The surgeon will then reposition the fat deposits, tighten the muscles and tissue and/or remove the excess skin. With lower eyelid surgery, the incision may be made below the lower lash line to remove excess skin. An alternate technique for lower eyelid surgery is a transconjunctival incision, which is hidden inside the lower eyelid and allows for the redistribution or removal of fat.
